This on-line form provides polling place information based on the address you submit. However, it does not confirm that you are a registered voter.

Vote by mail
You can vote from the comfort and convenience of your own home! Follow the steps below to keep voting as simple as it should be.
Before you request your vote by mail ballot:- Are you registered to vote?
If you haven't registered yet, fill out a voter registration form and mail it to your County's Board of Elections. [Deadline: Must be received by May 13] - Are you unaffiliated or not sure if you're registered as a Democrat?
Fill out a party declaration form and mail it to your county Board of Elections. Unaffiliated and newly registered voters may declare up to and on the day of the election.
How to vote by mail
- Download and print an absentee ballot application.
- Fill in (or correct) your personal information at the top of the vote by mail application.
- Check the “Primary ” box to request a vote by mail ballot for the June 2008 election.
- You do not need to choose an Absentee Voter Option. This can be left blank if neither option applies to you.
- Fill in the correct mailing address where you would like to receive your ballot. If this is the same address at the top of the page, you can leave this section blank.
- Remember to sign and date your ballot application where indicated underneath the mailing information. Your application will not be accepted without a signature.
- Fill out the return address in the upper left-hand corner on page 2 of the application.
Mailing in your application
If mailed, your application must be received by the County Clerk's office by May 27th. You may also apply in person to the County Clerk until 3pm the day before the election. The County Clerk cannot accept faxed copies of an Absentee Ballot Application since an original signature is required.

If you apply for an absentee ballot on time but don't receive it by election day, you can vote by provisional ballot at your polling place.
